Obtaining a Sick Note
You self-certify for the first 7 days, as the GP will only provide a sick note from day 8 of your illness.
You can complete a self-certification form online at https://www.tax.service.gov.uk/fill-online/statutory-sick-pay-employer-notification?_ga=2.241364994.1037507623.1685699428-910408850.1674557266, or should speak to your employer if they have an alternative form that they would prefer you to use.
If you are off sick for longer than 7 days, please complete an eConsult https://goldingtonavenuesurgery.webgp.com/react-consult-administrative-help?codeName=GEC_ADM
If you are not able to use the online form, please phone and talk to our patient services team who will take your information and ask our duty GP to provide the certificate.
We will usually text you a pdf of your ‘sick note’, so that you can then forward this on to your employer or print this out at home – if you want to pick up a printed copy from the practice instead, please let us know that at the time that you are making your request.
In most instances we are able to provide the certificate based on the information that you provide, but in some instances further information or an appointment may be required but we will let you know if that is the case.
We will usually send you a text message (if we hold your mobile number) to let you know the outcome of your test results, and whether you need to book an appointment to follow up.
You can view your test results online, if you have ‘detailed coded records access’ – if you don’t have this access already, this is really easy to set up and you just need to come to the practice (either of the buildings) with your identification. We need to see your driving licence, or your passport and a utility bill. Then we can get this set up so that you can view this online.
If you don’t have online access but would like to know the results of tests that we have ordered, please telephone us and select option 1, the patient services team will be able to tell you what your results are, but not interpret them for you.
If you have had tests that were ordered by a hospital or other clinical team, we will not be able to give you these results. Results go back to the person or team that ordered them, and are their responsibility to action and communicate the results to you.
