Request an Appointment or Medical Help Online
Use SystmConnect to request an appointment, ask a clinician to review a medical problem, or get help with a health concern. You do not need to log in to use this service.
Simply click Start SystmConnect below and answer a few questions about your symptoms or concern. This helps us direct your request to the right member of the team and ensure you receive the most appropriate care.
Before you submit
- You can use SystmConnect without logging in.
- Logging in helps us identify you more easily and allows you to upload photos or documents.
- Online requests can be submitted Monday to Friday, 7:30am–6:30pm.
- If your problem is urgent and it is after 4pm, please telephone the surgery instead.
- If you are requesting a home visit, please submit your request before 10:30am wherever possible. Home visits are reserved for patients who are genuinely housebound or too unwell to attend the surgery.
What happens next?
Your request will be reviewed by our Care Navigation Team, who work closely with our clinical team to ensure you receive the right care.
We will usually contact you the same day by text, telephone or email to let you know the next steps. Depending on your needs, we may:
- Ask a clinician to review your request.
- Arrange an appointment.
- Send you a booking link.
- Signpost you to a more appropriate service.
If your symptoms change
Please submit a new SystmConnect with the updated information and let us know it relates to an existing request.
If you cannot complete SystmConnect
A friend or family member can help you. Alternatively, please contact the surgery and a member of our team will assist you.
Need urgent help?
If your condition worsens while you are waiting for a response, please contact the surgery. When the surgery is closed, contact NHS 111 for urgent medical advice.
If you have a life-threatening emergency, such as severe chest pain, difficulty breathing, symptoms of a stroke, or loss of consciousness, call 999 immediately.
